Abacus Care Coordination is a new firm which focuses primarily on principles of collaboration, supporting persons who recognize the need and benefit of an active and dynamic system of natural and programmitic supports.   With the support of one's ever expanding community, people who are "differently abled" can achieve the same degree of independence and inter-dependence as anyone else. 

Abacus helps waiver service recipients really build community, not only to obtain assistance from, but to lend assistance to.  Because one is seen as or considered "disabled" does not mean they are limited, does not mean that the world becomes "closed" and certainly does not mean  "less deserving".

Conflict Free Case Managment

Since this was first announced there has been confusion about this.  If you or a loved one received waiver based services of any kind this matters and you need to know a few important details.

 

  1. The purpose of CFCM is to insure that the firm, person etc. that advocates for your or your loved one is free from any obligation to a service provider.
  2. The intent of these rules is to better provide an unbiased perspective of your Care Coordinator related to the suggestions he or she may make for service providers.
  3. The short term result has been chaotic, yes.  It will level out in the next year to 18 months.
  4. The "Requirement" that Care Coordination services be separated from other services takes effect July 1, 2016.  This means that your care coordinator may not have any affiliation with a service provider beyond this date.  ANY!
  5. Find a Care Coordinator.com only lists those Care Coordinators that have no affiliation today.  This means that if you select a Care Coordinator from this site you will have no surprises come July 1, 2016.

With all the turmoil that's going on we thought you might like to have a simple place where you could get the contact information for Care Coordinators that have openings on their caseload.  The quicklist from Alaska SDS is nice and pretty and all but there's no information there about who has room on their caseload.  Subsequently, if you're looking for one that has some room you must call thru the list until you find one.  With so many on the list and most of them plenty busy, just getting an answer can be hard. 
We're here to help straighten this out. 
Here you'll find an alphabetical listing of Care Coordinators that have told us they have room on their caseloads for more people.  We'll also get information on where they operate, what kind of waivers they prefer to work with and any other specifics that will be of assistance.  We're glad to do this because we believe in the system we have here in Alaska and think that our people deserve the best.
